ICSA Model Declaration of Interest Form (2007)

A declaration of interest form serves to ensure the integrity and professionalism of all staff, volunteers and trustees, who are expected to act in the best interests of their charity all times.

There will, however, be occasions when an individual has more than one legitimate interest. Such interests may conflict with those of their charity. In order to protect themselves and their charity, staff, volunteers and trustees should always declare a conflict of interests at the time it arises.
